Modivcare Transportation FAQ

How do I get started and what phone number should I call?

  • Feel free to contact Modivcare directly and let them know you would like to get set up with transportation assistance: 844.549.8353. 

  • If you prefer, you can call Seed Sower Transport and we will help with the process: 304.254.9928.

What information do I need to register with Modivcare?

  • Your first and last name, date of birth and phone number

  • Health care plan and member ID # (you can find this on your health plan card)

  • A few other details re type of transportation needed, location for pickup/droff off,  and level of service.

What information do I need to register with Modivcare?

  • Each health plan is different, so you may want contact your plan to confirm and understand your transportation benefits.
